I am raising this complaint regarding a failed/successful UPI transaction where the amount has been debited from my bank account but was not credited to the intended recipient. The transaction was done on [Date] for an amount of ₹[Amount] using [UPI App – e.g., PhonePe / Google Pay / Paytm]. The transaction details are as follows:

Despite waiting for the usual time frame and contacting the app/bank’s customer support, the issue remains unresolved. This has caused financial inconvenience and mental stress. I request NPCI to kindly investigate this matter and initiate a refund or settlement at the earliest
